Part II of the University of Pittsburgh Student-Athlete Agent/Advisor Registration Application is outlined below. Please certify your application and submit all required paperwork to adamsoles@pitt.edu. Your application will not be considered final until all parts of the registration process are complete.

REPRESENTATION DOCUMENTATION
PROFESSIONAL SPORTS EXPERIENCE

***Please attach a copy of your current, valid registration for each Players’ Association in which you are certified.

***Please attach a list of all athletes, coaches, and sport administrators that you represent or have represented in the past five years in contract negotiations. Please include on the list each person’s address, phone number, the dates of such representation, their sport, and the team(s)/organization(s) involved.

Please advise in a separate paragraph on the attachment whether special circumstances exist such that you prefer certain former clients not be contacted, and describe those circumstances. If any of your current clients expressly object to their address and telephone number being released, you may withhold this information with the understanding that, on request of a University of Pittsburgh Student-Athlete, you will arrange for a client or an intermediary to contact the student-athlete or a University official for purposes of serving as a reference for you.

In all other cases, the filing of this registration indicates that you agree that the University of Pittsburgh may contact anyone on your client list to discuss your contract services as an agent.

***Please attach the name, address, phone number and organizational form (specify sole proprietorship, corporation, partnership or other entity) for each firm or organization with which you are currently affiliated and indicate where the business of representing professional athletes is customarily conducted. Describe fully the nature of each of your firm(s) and organization(s) listed.

If any entity listed is a partnership, list the name of each partner. If any entity listed is a corporation, list the name of each officer and member of the board of directors. Please designate those partners, officers or members of the board of directors who customarily perform work for professional athletes.

Further, list each person not named in this section who: (a) has a significant ownership interest in your firm or organization; (b) has wholly or partially financed your firm or organization (other than financing or credit extended in the ordinary course of business by lending institutions); or (c) directly or indirectly exercises or has the power to exercise a controlling influence over the management of your firm or organization. Describe the ownership interest, amount of financing, and/or basis of controlling interest.

List each person employed by, volunteering for, consulting with, or interning for your current business and include their names, services provided, training, experience and specialty.

MISCELLANEOUS

***Please attach your representation contract, your resume (if available), and any other materials about you and/or your organization which you believe will benefit the University of Pittsburgh Student-Athletes and the Agent & Advisor Program. Please also include photographs of all individuals (including yourself) employed by, affiliated in a professional capacity with, or who may represent you or your organization in dealings with Pitt Student-Athletes. Each individual’s respective name, cell phone number, and email address should be attached to their photograph.

Please list all social media accounts (including Twitter, Facebook, Instagram, etc.) affiliated with or used by you and individuals who may represent you/your organization.

REFERENCES

***Please attach a list of three character references who are not related to you, not engaged in business with you, have known you for at least three years, and can attest to your character. For each, provide name, address, phone number and relationship.

Please attach a list of at least two entities that can attest to your financial credit. For each financial reference, provide the name of the organization, contact person, address and phone number.
